I do consulting with a medical billing office, for Anesthesiologists. He does use some remote workers, but everyone of them worked in his office at least 3 to 6 months prior to moving off-site.

The local hospital here, also uses remote coders, but again, all of them worked in their offices for a period of time first.

In both cases, the workers were leaving due to school, pregnancy, moving out of the area, etc., and had proved themselves very fast, and very accurate, so were given these opportunities to continue working.

I'm guessing there are smaller medical offices that may outsource coding to individuals, but larger offices will be very careful, since they are the ones remaining liable.
