a description of the image. if you can make it a keyword also, fine. otherwise, use the "title" tag for your keywords.

I would suggest using it, and as intended (to accurately describe the image). There is the possibility that Google may slightly favor the site that is more "accessible." But if nothing else, do it for the poor blind guy. With that in mind, please don't keyword stuff - dude doesn't need to hear 50 keywords being rambled out . . .
As far as using it for image search, I only know of one industry where people looking for pictures have a real possibility of turning into a customer.
