Title should be short and descriptive with your keywords on it...
Don't have the same title element on every page. Variety is the spice of life and, combined with relevance, is a pre-requisite to avoiding duplicate content issues and Google's supplemental index.
Meta tags now are being ignored by SEs some still uses it especially description....Google sometimes takes description from the site's contents..
Don't stuff keywords in your meta tags...
